Hockey Queensland Statewide Insurance is provided by HonanSport.  Following is the information you require should you sustain an injury and wish to make a claim via your hockey insurance.  

PLEASE NOTE: You must register your injury within 30 days from injury occurring and then you have 12 months to make a claim.  Please ensure you also complete an Incident Report which can be located from the Committee Members on Duty or Emailing Committee Members to get them email you the form
